Foster Care Specialized Foster Care (SFC) is designed to provide an alternative, less restrictive family setting for children who would otherwise be placed in an institutional setting. The goal of the Specialized Foster Care Program is to provide specialized intervention in a least restrictive, normalized family environment that prepares the client for family reunification or successful living in a less intensive setting. All potential SFC families are required to complete 24 hours of preservice training: ongoing training for licensed families is also required. Each child in the SFC program is provided a social worker who facilitates progress towards the child's goals as identified in a treatment plan. Initial treatment plans are developed within 24 hours of the child's admission and completed within 30 days. Treatment Plan Reviews are held quarterly to assess progress. All who work with the child are part of the team and play a vital role in Treatment Plan Reviews. SFC social workers monitor and facilitate the placement of children, provide specialized services to students and foster families and support interactions between primary team members. Social workers visit weekly with the child and SFC family. Social workers are available to SFC families 24 hours a day, seven days a week. SFC families are encouraged to utilize Elon Homes' respite services to help alleviate the demands of specialized foster care. SFC parents provide 24 hour care and direct child supervision as well as implementation of the child's treatment plan. |
