Family Care Program

Elon Homes for Children Family Care Program is a family centered program which has been developed around the risk and resiliency model of treatment. By providing education, support, and advocacy to at risk families within their own communities and home environments, we hope to increase family resiliency. Ultimately, the Family Care Program strives to successfully reunify families and to build stronger families with positive life outcomes.

Goals and Objectives:

  • Maintain relationships with students and their families.
  • Provide and arrange services for families.
  • Develop and enhance support for families through the coordination of services within the community.
  • Provide an individualized plan of care for families, based on risk and protective factors.

The Family Care Program serves families that:

  • Have a child placed at the Kennedy Campus of Elon Homes for Children.
  • Have a child attending the Kennedy Charter Public School.
  • Are in need of services while under DSS/YFS supervision.
  • Are at high risk for DSS involvement.

These are some of the services we are able to provide to at-risk families:

  • Training for parents, utilizing Active Parenting for Teens.
  • Facilitating teen groups with topics such as anger management, social skills, and other educational training.
  • Assistance in the coordination of services recommended by the court system.
  • Supporting family reunification through intensive in-home contacts.
  • Tracking family home visits.
  • Developing a cacomprehensive Plan of Care to meet the needs of the families in ourre.
  • Mentoring for teens.

The goals and objectives of Elon Homes for Children's Family Care Program are met through the following service elements:

  • Clinical Services - Assessment, Individualized Family Plan of Care, Case Management and Documentation.
  • Family Support - Parenting skills training, social support groups, on-call support 24 hours a day, supervision and support, and other education training as needed.
  • Service Relationships - Family Care will maintain a close working relationship with Kennedy Campus staff, referring county agencies, and other service providers to ensure the needs of the child and the family are being met.
